Consent Form (Personal Data Protection Act 2012)
个人资料同意书

As our donors, you are deemed to have given consent to Tzu Chi Singapore to collect, use and disclose your personal data for the following purposes:

1. To facilitate the collection of donations;
2. To process donations (and/or refund of donations in exceptional circumstances);
3. To verify your information and conduct anti-money laundering and other crime checks when necessary;
4. To administer the issuance of donation receipts;
5. To facilitate the submission of tax-deductible donation details to the Inland Revenue Authority of Singapore (IRAS);
6. To manage and maintain your donation records with us to fulfil document retention requirement by law;
7. To maintain donor database and perform donor giving analysis;
8. To respond to your enquiries and complaints;
9. For audit, risk management and/or compliance purposes; and
10. To comply with any applicable laws, regulations and guidelines.

In addition, your personal data shall be used, retained and disclosed for the following purposes, for which we seek your consent.

£ I consent to allow Buddhist Compassion Relief Tzu-Chi Foundation (Singapore) to provide updates on its fundraising events, other programmes, activities and news to me.

成为捐款者，即表示您同意新加坡慈济收集、使用、披露您的个人资料，作为以下用途：

1. 收取捐款；
2. 处理捐款相关的行政作业（包括个别特殊状况下允准的退款）；
3. 核对捐款者的身份和资料，以及在必要时进行反洗黑钱与其他犯罪查证；
4. 开发捐款收据；
5. 向税务局登记捐款估税扣税（符合资格者）
6. 管理与保存捐款资料，以符合资料保存的法定需求；
7. 维护捐款者资料库及进行捐款相关的分析；
8. 回复及处理您的询问或投诉；
9. 审计、风险管理、合规用途；以及
10. 遵守相关的法律与准则。

另外，我们也将使用、保存、披露您的个人资料，用于后续互动和联系，在此需要您的同意：
